These are just a couple of the things I saw that you should get rid of.

O2 - BHO: (no name) - {1C3C4699-B285-475F-BE47-0B26088CE876} - C:\Program Files\Video ActiveX Access\iesplg.dll (file missing)

O3 - Toolbar: (no name) - {F06E2ABE-3A50-4079-BE25-FC100D9EAA25} - (no file)

O4 - HKLM\..\Policies\Explorer\Run: [user32.dll] C:\Program Files\Video ActiveX Access\iesmn.exe

Those are just what I noticed quickly looking over it. Here are a couple handy sites to bookmark so you can analyze and future Hijack log files.
Maybe I missed them but it looks like you dont have a virus scan installed. Maybe you do and it didnt show up in the log file.

Here are the sites-Good luck. I use the second one more than the first usually just use the first as a double check.
